Yes, a dental implant with bone loss can often be saved. Whether it can be treated depends on how much bone support remains, whether the implant is still stable, what caused the bone loss, and the condition of the surrounding gum tissue.
Bone Loss Does Not Always Mean an Implant Is Failing
Dental implants depend on the surrounding jawbone for support. When that bone begins to recede, the implant has less tissue holding it in place.
The significance of bone loss varies. A small change in bone level is different from progressive loss that extends farther down the implant. An implant can also remain stable even when X-rays show that some supporting bone has been lost.
This is why bone loss needs to be evaluated in context rather than treated as implant failure by itself.
What Causes Bone Loss Around a Dental Implant?
Peri-implantitis is an inflammatory condition affecting the gum and bone around an implant. Bacteria accumulate around the implant, and the resulting inflammation can progressively damage its supporting bone.
Several factors can contribute to peri-implant disease or make bone loss more difficult to control, including:
- A history of gum disease
- Plaque buildup around the implant
- Smoking or vaping
- Poorly controlled diabetes
- Difficulty cleaning around the implant
- Excess cement below the gumline
- Implant or restoration positioning that limits cleaning access
- Excessive bite forces or grinding
Identifying the specific bacterial cause is an important part of treatment because stopping further bone loss requires addressing the conditions contributing to it. We use microscopic monitoring and often cultures to identify this cause and prescribe the appropriate antibiotic.
Can Bone Grow Back Around a Dental Implant?
In some cases, it may be possible to regenerate bone around a dental implant, but not every bone defect is suitable for regenerative treatment.
The potential for bone regeneration depends on factors such as the shape and extent of the defect, implant stability, control of the infection, and the condition of the surrounding tissues. Regenerative procedures may use bone grafting or other techniques to encourage new support around selected implants.
Successful regeneration also depends on controlling the infection or other condition responsible for the bone loss.
How We Determine Whether an Implant Can Be Saved
There is no single amount of bone loss that determines whether every implant can or cannot be saved. The overall condition of the implant and surrounding tissues provides a more complete picture.
We evaluate factors such as:
- Implant stability: An implant that remains firmly integrated into the jaw generally has a different prognosis than one that has become mobile.
- Remaining bone support: X-rays and clinical findings help show how much bone remains around the implant.
- Pattern of bone loss: The location and shape of the bone defect can affect which treatments are possible.
- Inflammation and infection: We look for bleeding, drainage, deep pockets, and other evidence of active peri-implant disease.
- Implant position: The position and angle of the implant can affect treatment access and long-term maintenance.
- Gum tissue: The health and amount of tissue surrounding the implant can influence treatment planning.
- Ability to keep the implant clean: An implant that cannot be cleaned effectively may remain vulnerable to recurring inflammation.
These findings help determine whether treating the existing implant offers a reasonable prognosis or whether another approach should be considered.
When Does a Dental Implant Need to Be Removed?
An implant may need to be removed when it no longer has enough support to function predictably or when the underlying problem cannot be successfully controlled.
Implant mobility is an especially important finding because a dental implant should remain firmly integrated with the jawbone. Severe loss of supporting bone, an unfavorable implant position, or disease that continues despite treatment may also make removal the more appropriate option.
If removal is necessary, the site can then be evaluated to determine whether bone grafting and future implant replacement are possible.
How Bone Loss Around an Implant Is Treated
Treatment depends on what is causing the bone loss and how much support remains around the implant. The first priority is controlling infection and inflammation to help prevent further damage.
Depending on the condition of the implant, treatment may include decontaminating the implant surface, addressing contributing factors, laser treatment for peri-implantitis, or regenerative procedures to rebuild lost bone.

For implants affected by peri-implantitis, LAPIP laser treatment provides a minimally invasive way to treat infection around the implant.
LAPIP uses laser energy to target diseased tissue and bacteria around the implant without traditional cutting or sutures. Treatment is designed to reduce infection and inflammation, preserve healthy surrounding tissue, and stimulate new bone growth to help restore support around the implant.
LAPIP may be an option when an affected implant still has enough stability and support to treat. Whether it is appropriate depends on the individual implant and the extent of the disease.

The rate varies. Peri-implantitis can progress at different rates depending on the infection, individual risk factors, oral hygiene, and other conditions around the implant. Regular examinations and X-rays help determine whether bone levels are changing over time.
Bone loss and peri-implantitis do not always cause pain or obvious looseness, particularly before the implant loses enough support to become mobile. An implant can therefore feel normal even when an examination or X-ray shows a significant problem.
Not always. Some people develop tenderness or discomfort when chewing, while others have little or no pain. Bleeding, swelling, drainage, recession, or changes seen on an X-ray can indicate a problem even when the implant does not hurt.
A replacement implant may be possible after a failed implant is removed, but the site must first be evaluated for remaining bone, infection, and gum health. Bone grafting or additional healing time may be needed before another implant can be placed.
